Smokin’ Poll: What “Rules” Do You Follow When Tipping?

Do you calculate a tip?  Do you just throw whatever you have left on the table?  Do you really change your tip that much based on the quality of the server?  If you're drinking do you tip with each drink or only at the end of the night?  There's a lot questions when it comes to tipping, which ones do you feel strongly about.  So tell us "What rules do you use when tipping?"
